Form A

  • reports projected information for the full term of the agreement

  • a one-time report, submitted to OSC with the contract for approval

  • submitted by the agency to OSC

Form B

  • a historical report for the most recently concluded state fiscal year

  • due annually by May 15 for each year that the contract is active

  • prepared and submitted by the Contractor to OSC, the contracting agency, and Civil Service
