Microsoft Forms and Lists Request Intake System

Presentation Summary.
This presentation will describe how I've used MS Forms, MS Lists, Power Automate to create an intake request system to screen software requests before determining if they are actionable and aligned with our strategic priorities. We will describe why we use this system, what the benefits and organizational impacts have been, and we will walk through the steps we followed to create the system so that attendees can create and customize their own if they so wish. We will provide details on the specific flows we use to trigger email notifications, and to transfer the information entered into the MS Form to the MS List. We plan to provide a 20-minute presentation with 10 minutes for Q&A.

Presenter Bios:
Chris Hurwitz is an experienced project manager with deep experience designing, managing, and advising on a variety of energy efficiency programs and technologies. At NYSERDA, Chris is leading the develop of several large efforts to scale program management and customer engagement systems, as well as helping several teams implement the Inflation Reduction Act Home Energy Rebate Programs in New York State. Prior to working for NYSERDA, Chris worked in the energy efficiency industry for 8 years, designing and selling commercial energy efficiency projects, managing energy efficiency portfolios for electric and gas utilities, working with software development teams to design custom software for commercial, and residential energy efficiency programs.

Rafi Zaman is a skilled business analyst with hands on experience in gathering and documenting requirements, creating detailed process diagrams as flowcharts, and producing key artifacts including Business Requirements Documents (BRDs) and Functional Requirements Documents (FRDs). He brings a strong background in User Acceptance Testing (UAT) and excels at facilitating communication across cross-functional teams to drive alignment and clarity throughout the project lifecycle. Prior to this current work, Rafi gained valuable experience in the financial service and healthcare industry, where he supported a range of initiatives focused on improving operational efficiency and implementing business technology solutions. His ability to translate complex business needs into actionable technical requirements has been instrumental in delivering impactful, stakeholder aligned outcomes.
